I've got a test running now, tracking temperature, cpu frequency, throttling status, and fan status, using ejolson's pichart lorenz code as a half hour stress test, see viewtopic.php?t=359885&start=25#p2160192. I'll post the results when completed.The official Pi 4 fan has no tacho output so no speed monitoring. It is PWM capable according to the product brief but the stock config uses the gpio-fan overlay which is only binary on/off control with a single temperature set point.
/sys/devices/virtual/thermal/cooling_device0/cur_state should show whether the fan is on or off.
